Thank you for your interest in subsidized child care.

Subsidized child care services are ONLY available for children who are citizens or legal residents of the United States.

To receive subsidized child care, your family must document an approved reason for needing child care and meet income eligibility criteria.

If your spouse OR the second parent OR another person legally responsible for the child(ren) in need of child care is residing with you, this person must also document an approved reason for needing child care and his/her income must be included when determining whether your family meets the income eligibility criteria.

For each question, answer Yes or No

 
 Is at least one child who is in need of subsidized care a legal resident of the United States ?

 Are you residing with a spouse OR the second parent OR other person legally responsible for the child(ren) in need of child care ?

 Are you in receipt of Temporary Assistance AND at least one of the following statements are true?
  • You are employed.

  • You are participating in an approved work activity.

  • You are participating in an approved school program.

  • You are participating in an approved vocational program.

NOTE: Temporary Assistance includes Cash Assistance, Temporary Assistance to Needy Families(TANF) or Safety Net Assistance to Families(SNFA). Please check "No" if you are ONLY receiving Medicaid and/or Food Stamps (SNAP).
